MEDICAL WAIVERS:
Recipient athletes and living donor athletes must complete a medical waiver in order to participate in their selected athletic events.
Medical waivers can be found on the Transplant
Games website and it's also be attached for your convenience.
YOU MUST HAVE YOUR COMPLETED WAIVER TO NICOLE SISEN BY JUNE 11, 2010.

The form must be clear, written legibly and must be completed. There is one spot on the form that EVERYONE has missed. Your physician must circle the letter A, B or C indicating restrictions.
They must AGAIN fill in the letter A, B, or C above their signature line.
Physician information must be completed in full detail.
Your team name is: Team Illinois
Your team manger is: Mary Schlereth
If the form is not completed correctly or clearly, it will not be accepted. Although tedious, this is a very important form that must be completed accurately for participation in the Games.
